Rental Pavilion #1

The undersigned agrees to reimburse Coleman Memorial Park for any property damage or breach of rules for pavilion rental incurred by utilizing the facility. The undersigned also agrees to comply with all posted rules and regulations for the facility and Park, Rules for Pavilion Rental, and City of Lebanon Park Rules and Regulations, Section 943.01 of the Codified Ordinances. The undersigned agrees to indemnify and save harmless Coleman Memorial Park and the City of Lebanon against any and all costs, actions, claims and demands whatsoever that may result from the activities and/or events conducted by or under the sponsorship of the undersigned. Any authorized Coleman Memorial Park or City of Lebanon representative reserves the right to terminate the rental and/or event immediately with no refund to the applicant.

Alcohol is prohibited. The use of piñatas is prohibited. Digging in the grass is not permitted; however, horseshoes/quoits are permitted in non-grassy areas. Trimming of bushes, shrubs or trees is prohibited. No person is permitted to carry any kind of lighted pipe, cigar, cigarette or any other lighted smoking equipment, including electronic cigarettes or similar devices, within 25 feet of athletic fields, athletic courts, and playground areas. The use of inflatable bounce houses, trampolines, and similar equipment is prohibited unless a hold harmless agreement and a certificate of liability insurance in a minimum amount of $500,000 naming the City of Lebanon and Coleman Memorial Park Board of Trustees as an additional insured is submitted to the City of Lebanon Department of Administration no less than seven (7) days prior to the event.
