Pickleball Tournament Registration at Coleman Memorial Park

By submitting this registration, I acknowledge that participation in the Coleman Memorial Park pickleball tournament involves physical activity and may carry a risk of injury. I voluntarily assume all risks associated with participating in this event, including but not limited to falls, collisions, and weather-related hazards.

I hereby release and hold harmless Friends of Coleman Memorial Park, Coleman Memorial Park, its organizers, sponsors, volunteers, and affiliates from any and all liability, claims, or causes of action arising from my participation in this event.

I understand that a full waiver and release of liability will be provided and must be signed on the day of the event in order to participate.
